Change the title of forms in the medical notes

The doctor would like to have the choice to change the title of the forms once it's in the patient file.

Memo and/or Alerts & Special Needs

This ONLY pops-up when the patient chart is accesses. But, if opening an appointment (to change the state or room) the users would like the alert to show there as well. If Memo had a checkbox option to "show on appt" that would be super helpful!
