Lisa Malenfant from "mylelan" would like to be able to see if an orphan patient is enrolled with the GAMF the same way she used to in Kinlogix.She'd like to have that new status in MYLE.

Patient index - The note in the patients contact should appear in the banner

When the users write a note under a phone number (Ex: Mom, Dad...) It should appear in the banner next to the number. Especialy for pediatric patients, they need to know who they are calling without having to go in the patient profile tab.

Add the English version of the 4101 form (enrollment with a family doctor) to MYLE
