Add pages number when printing elements from the patient files

When printing the complete patient file, page # and total pages printed are mandatory to be sure nothing is missing. It could be interresting to add this kind of counter at the bottom : INFORMATION CONFIDENTIELLE - PAGE 1/100 for example

Karine Lord from mcstjean Sometimes the office assist go so fast and click by mistake on Approve in the note. Making them not able to approve notes, will avoid problem like this, as they do not even need to approve notes
