Add time in note printing

In the portfolio, there is the printer button to print a summary with all the notes. The date of the note appears but not the time. It was requested by Dr. Beshay's clinic to add it.

Memo and/or Alerts & Special Needs

This ONLY pops-up when the patient chart is accesses. But, if opening an appointment (to change the state or room) the users would like the alert to show there as well. If Memo had a checkbox option to "show on appt" that would be super helpful!

External Provider add to Banner

Allow the External Provider to show on the banner. option/checkbox to display on banner when needed. this is required for UPCCs to know the patient default family physician.
